Creator's Statement
As a clinical
psychologist with over 25 years of therapy experience, I have
taken notes on a lot of sessions.
Despite being technically savvy and having expertise with
dozens of applications for both PC and PDA, I had never felt a
need to computerize my notes; pen, paper and manila folder
worked fine. Until
now.
HIPAA changed
that. Now I was
faced with the prospect of maintaining two separate files to
take advantage of APA’s intensive efforts to establish the Psychotherapy
Note Exemption. And
I recognized that much of my PHI energies would not contribute
to the clinical care of my patients.
Data like prognosis, progress to date, diagnosis, and CPT
code, perhaps essential to a medical practice with several
hundred patients, were clearly familiar to me as a product of
the therapeutic relationship.
A customized
computer database was the solution.
I could maintain separate PHI
and Psychotherapy Notes, yet have fluid access to both.

PHI.
